• All payments have to be up-to-date for plan benefits

• You can cancel the plan with 30 days written notice. No refunds will be issued for part months.

• Payments will be collected on the renewal date of every month by via your selected payment option which you select during the checkout process.

• If you wish to pay by check, cash or bank transfer we would request you contact our offices directly to make the necessary arrangements.
